Personnel and Program Reports Requested

Thank you for your kind assistance to the Office of Faith Formation and Lay Ecclesial Ministry in maintaining accurate contact lists of parish personnel involved in leadership and on ministry teams as we strive to provide ongoing formation. By returning the updated personnel lists annually, persons are invited to formation appropriate to their areas of ministry. Additionally, a more accurate analysis of statistics is reported in diocesan, national and universal church data collection. Catechetical program reports are also requested. Dioceses are required to report actual numbers of persons served in formation opportunities for adults, young adults, youth, children, families, and for sacramental preparation. Please provide us with the opportunity to verify accurate numbers by completing the catechetical report, according to your area of responsibility. Please share information from others on your staff who oversee ministry areas beyond your position.
