Reference Manual & Forms
The Financial Procedures and Controls Manual is designed to provide financial guidelines to assist pastors, administrators, parish leaders and bookkeepers in the financial administration of diocesan entities. It provides procedures that will assist in safeguarding the entity’s assets through uniform methods and procedures.
The following manual and forms have been revised as of January 2023.
DOY Report Forms
To use these forms:
- Click the button to download the form to your computer. Print the form to fill out by hand.
- Some forms are fillable PDFs. Open the file using Adobe Reader to type in your responses. Download and install Adobe Reader from: https://get.adobe.com/reader/
- Click “Save As” to save the filled out form under a new name (for example add the date, your initials, etc.).
- Save frequently to avoid losing your work.
